Vascular parkinsonism is a heterogeneous syndrome characterized by parkinsonian
features due to cerebrovascular disease. While cognitive impairment is common in
vascular parkinsonism, neuropsychiatric manifestations such as visual hallucinations
and wandering behavior are infrequently reported. We present a case of a 72-year- old vulnerable female with vascular parkinsonism who developed recurrent visual hallucinations and episodic wandering, highlighting the need for comprehensive neuropsychiatric evaluation and holistic management in vascular parkinsonism.
